Waivers

Ready to Ride,  Complete forms prior to arrival

Lessons/Riding Required Form

Safety First: Required Forms
Your safety and the well-being of our horses are our top priorities. In compliance with state law, all riders, handlers, and visitors participating in activities at Sapphire Ribbons must have a signed liability waiver on file before entering the arena or handling any horses. Minor riders must have this form signed by a parent or legal guardian.

Horse Entry to Sapphire Ribbons

Prior to your horse arriving at Sapphire Ribbons, the Horse Entry Form MUST be completed. Whether you are trailing in for the day/boarding/enrolling in our training program the Horse Entry Form is required prior to arrival. To ensure a safe, organized, and seamless transition into Sapphire Ribbons, all incoming clients must submit their required paperwork before their first scheduled date. Along with our standard liability waiver, every horse entering our training or conditioning programs must have a completed Horse Information Sheet on file. This allows us to strictly follow your horse's specific feeding schedules, supplement needs, behavioral history, and vet/farrier preferences from day one.
